Private Care: VNA Caring Link - Geriatric Care Management

The VNA CaringLink Program is a comprehensive program comprised of a geriatric care management assessment, a concise plan of care, and concrete supportive services. The focus of VNA CaringLink is to plan effectively for the present and future needs of older individuals, provide and coordinate these services and continually assess their effectiveness.

VNA CaringLink is a program that fosters independence and quality of life for our geriatric clients. This is achieved through coordinated professional, para-professional and non-medical services that help individuals to remain in their homes.

We will guide families through the process of determining where to begin, what questions to ask and how to arrange for services. Our professional team of nurses, social workers and geriatric care managers can answer such questions as:

  • What services and community resources are available in the community to best fit my family member or client's needs?
  • Would my family member or client qualify for services which are covered under medicare or medicaid?
  • How can long term care insurance or private pay resources be utilized for the most cost-effective plan? 
  • What help can the VNA Home Health Services provide to assure that appropriate and intergrated services are received? 

How can we handle managing the non-medical needs of daily activities, home maintenance and socialization?

How the consultation service works:
The VNA CaringLink is comprised of three components: VNA CaringLink Assessment, VNA CaringLink Continual Care and VNA CaringLink Choice. A family member, friend or other responsible party may request a consultation. The initial evaluation is provided free of charge at the VNA HHS Office or by telephone.

VNA CaringLink Assessment:

The first step is understanding the individual. What services would benefit the client most? Our comprehensive assessment will assess the client’s physical, mental, social, economic and functional abilities in the home environment. It is performed by a registered nurse and a social worker or geriatric care manager. The assessment also identifies appropriate setting/needs for the client and makes recommendations to the family via the care plan. The final service of this assessment is to connect the client and their family with the recommended resources.

The nurse and geriatric care management specialist will help to:
  

 

  •  Assess the senior's current needs and situation, involving medical, social or personal care needs, or the home environment.
  • Identify community programs and resource options that can address identified needs. 
  • Formulate an action plan.
  • Help the family make contact with appropriate services.

If the client feels that an additional consultation is needed, the VNA HHS team is available to make an in home visit on a fee basis for a more in depth comprehensive evaluation and services plan.

VNA CaringLink Continual Care:

CaringLink Continual Care is a total plan of care that fosters independence and quality of life in the home, based on the assessment and the requests of the client and their family. The effectiveness of the plan is monitored and assessed.

VNA CaringLink Choice:

Services can be requested individually or as a group. They are based on the geriatric care management team recommendations, the client’s request or family needs.
